I regret very much to inform you that he expired on____________ and accordingly Xerox copy of death certificate is enclosed. With his death since he is no longer the subscriber of the said telephone, the undersigned being the widow of my deceased husband would request you to arrange to change the subscriber’s name of the telephone in my favour.
As a token of proof that I am the legal heir after his death, Xerox copy of our marriage registration certificate is enclosed.
