Dear Sir,
While I was staying at____________ (Place) I was drawing ration against 5 (five) Ration cards from there in respect of myself, my wife and three sons. I along with the members of my family have shifted my residence at the above address very recently for staying here on permanent basis.
At the time of leaving that place I surrendered and deposited all the five Ration cards with the office of the Rationing Officer____________ (Address) against receipt (Date) ____________ issued by them, Xerox copy of which is enclosed.
I would request you to arrange to issue fresh Ration cards in lieu of the above Ration Cards, at your earliest convenience. The names and addresses with other details to be mentioned in the Ration Cards to be issued are as follows:
